Students who are seeking a major in Aging Studies (Gerontology), Communication Sciences and Disorders, Criminology, and Social Work must declare or change their major via the new College website at http://www.bcs.usf.edu/CurrentStudents/UndergradForms.cfm.
Change of Catalog
As long as you have been continuously enrolled at USF, you may change
your catalog year to any year within the time you have been continuously
enrolled.
Change of Major
This process is available online and is used for current major in
the College of Arts and Sciences. With this form, you can either change
to a different major (within our College) or add a second
major within our college.
Cross
Enrollment or Transient forms Coversheet
This form is used by students wishing to attend another accredited
institution of higher learning either as a Transient or Cross-Enrolled
student. It is a questionnaire designed to determine the appropriateness
of your courses. Upon completion, please mail or hand-deliver to BEH
201. This form must be accompanied by either the Transient or Cross-Enrollment
form for processing. If you are a student receiving financial aid,
you should visit Financial Aids website to download the appropriate
form to receive monetary aid for your courses. The Financial Aid form
must also be turned in with your Transient or Cross-Enrollment form
to our offices for approval.
Declaration of Major
for Current Degree Seeking Students
This process is available
online and is used for student's whose major is "outside" of
our College.
Declaration of
Major for Former Student Returning
If you have an "ST" hold
on your record and you are returning student, out of the University
of South Florida for over a year, please contact the Office of Orientation at 813-974-3060.
Double Majoring Between Two Colleges
If you are interested in double majoring between two Colleges (i.e. Chemical Engineering & Mathematic or Accounting & Public Relations, and/or, etc.) complete this form. Do not complete the online declaration of major. Please complete this form and turn it in to BEH 201; or fax it to (813) 974-4075.
Graduation
Verification Letter Request Form
This is to request an official letter stating that you have completed
or are about to complete all requirements for graduation. You may mail/hand-deliver
this form to BEH 201 or fax it to (813) 974-4075. This is ONLY for
undergraduate students. If you are a graduate student, please contact
your department for a letter.
Leaving or Dropping Your CAS Major
If you are interested in changing your major to another College (i.e. Pre-Education, Accounting, Engineering, Undecided, and/or etc.) complete this form. Do not complete the online declaration of major. Please complete this form and turn it in to BEH 201; or fax it to (813) 974-4075.
Overload
Students are allowed to take a maximum number of 18 credit hours per
semester in the fall and spring and 14 credit hours in the summer.
If you wish to go over that amount of credit hours, you will need this
form. If your GPA is below 3.0, you will need to have your academic
advisor sign this form. If it is above 3.0, you can just mail or turn
it in to BEH 201; or fax it to (813) 974-4075.
Request
an Update to SASS Form
If you believe there are mistakes within your SASS report, this is
the form you'll need to make changes. Print it out, explain the mistake
and mail/hand-deliver to BEH 201 or Fax it to (813) 974-4075.
S/U
Contract
If you wish to take a course in exchange for an "S" (Satisfactory)
or "U" (Unsatisfactory) grade. It
is considered a private agreement between you and the professor and
does not need to be submitted to anyone. Simply have the professor
sign and make a copy for each other. "S/U" grading does not
affect your GPA at all. Check with your advisor to find out whether
certain classes can be taken "S/U". This form is available in the department offering the course.
Time
Conflict
Sometimes course times overlap and OASIS will not let you register
for them. This form needs to be signed by the professors of both classes
and also require a departmental stamp. This completed is turned in
to BEH 201 if you are a declared major in the College. The system should
allow you to register within 24 hours of submission.
